BIOLOGY AND BUTTERFLIES LLC PROGRAM WAIVER & CONSENT AGREEMENT

1. General Participation & Risks

This seasonal enrichment program includes weekly sessions with live insects, garden tools, seeds, soil, craft materials, group movement games, and outdoor exploration. Each session is designed for ages 3½–11 and focuses on science learning through sensory-based, hands-on activities. I understand that: - Activities may include handling caterpillars, chrysalises, and eggs - Children may interact with natural materials such as dirt, clay, kinetic sand, seeds, and water - Children may engage in physical games involving crawling, running, and lifting lightweight materials - Materials such as scissors, glue, paint, biodegradable pots, and garden tools may be used under adult supervision - Outdoor sessions may include exposure to insects, bees, butterflies, uneven terrain, sun exposure, and mild environmental allergens I accept the inherent risks of participating in this type of outdoor, interactive, science-based programming.

2. Medical Conditions, Allergies, and Sensory Needs

I confirm that: - I have disclosed any medical conditions, allergies, or sensory sensitivities on the registration form - I understand that while staff will take reasonable precautions, Biology and Butterflies LLC is not liable for adverse reactions due to undisclosed conditions Special notes: - Kinetic sand, biodegradable materials, soil, live plants, seeds, flowers, and non-toxic craft materials are used weekly - Children may be offered optional contact with clean, enclosed insects (e.g., caterpillars in magnifying jars)

3. Developmental Readiness & Behavior

This program is milestone-based. Children must be able to: - Participate in small group instruction - Follow gentle guidance - Transition between activities - Safely interact with peers and materials I understand that if a child displays consistent unsafe, aggressive, or disruptive behavior, they may be removed from the program without refund to maintain a safe environment for all participants.

4. Non-Refundable Policy

I understand: - All program payments are non-refundable - This includes missed days, illness, behavioral removal, weather cancellations, or voluntary withdrawal for any reason - Once registered, my child’s spot is held and supplies are prepared specifically for them

If a child is withdrawn after enrollment, the payment will be retained and applied toward ongoing costs of the program, including care of our live butterflies, vivarium upkeep, and pollinator garden plants. This supports the continued operation and education mission of Biology and Butterflies LLC.

5. Photo and Video Consent

I grant permission for my child to be photographed or recorded during program activities for the purpose of: - Educational documentation - Program marketing (e.g., flyers, social media, website) - Reporting and grant support for pollinator outreach No names will be shared publicly. If I wish to opt out of media use, I will email biologyandbutterflies@gmail.com before the program start date.

6. Acknowledgment and Consent

By submitting this form and checking the agreement box, I acknowledge that: - I have read and agree to all waiver terms - I accept all risks associated with participation - I grant consent for medical/emergency contact use if needed - I accept that no refunds will be issued once registered - I affirm that my child is developmentally ready for this program or that I have reached out for special approval

7. Exposure to Native Plants

Children may come into contact with the following Texas native host and nectar plants during the program. These plants are used for education and butterfly conservation purposes. If your child has a known allergy to any of these, please notify us in writing before the program begins: - Rue - Dill - Fennel - Milkweed (including native species) - Passion vine - Dutchman’s pipevine - Pipevine - Spicebush - Camphor tree - Pawpaw tree

8. Photo and Video Permissions (Select One during checkout)

By checking the box at registration, I confirm my permission preference regarding photos and video taken during the program. These images may be used for the purposes selected below:
